If you are a new client, we welcome you to Piedmont Pets and thank you for entrusting us with your pets! Before coming to your first appointment, you may complete our New Client form * or you can fill it out upon your arrival.

Boarders
If you are boarding your pet, please complete our Boarding Agreement * and bring it with you to our office. Thanks!
